Making a workers compensation claim

If you have any time off work or need any medical treatment because of an injury at work you should complete a workers compensation claim. 

To do this you should:

• complete a workers compensation claim form which your employer should provide
• obtain a workers compensation medical certificate from an accredited doctor 
• give both the completed claim form and medical certificate to your employer.  You should keep the 'worker's copy' of both the claim form and the certificate as your claim will not be complete until you provide both these forms.
